COMMERCIAL INVOICE:
When creating the commercial invoice please use HS code 9801.00.1012 for items to
be returned. It is important to include a note on the commercial invoice stating:
“US GOODS FOR REPAIR AND RETURN”

1. Importer’s Declaration of US Goods Returned
2. Foreign Shipper’s Declaration of U.S. Goods Returned
3. Completed Commercial Invoice

*PDI will not be responsible for any charges related to the shipping or import of repairs or recalibrations. All
shipments must be sent in as DDP incoterms. Any charges to PDI will be assessed to the client with repair
costs prior to the product being shipped back.
